You changed it an hour ago. It still leaked.

If that sounds familiar, you’re not dealing with a fluke,  you’re dealing with a fit or absorbency problem that has a fix. Leaks from adult diapers are common, but they’re rarely random. There’s almost always a specific reason, and once you identify it, the solution is usually straightforward.

Here’s what’s actually causing the leaks, and what to do about each one.

The Two Root Causes of Almost Every Leak

Before getting into specifics, it helps to know that nearly all adult diaper leaks come down to two things: wrong fit or insufficient absorbency. Everything else is a variation of one of these two.

1. The Fit Is Off

Around half of all leaks from incontinence products happen because of incorrect fit. And the mistake goes both ways: 

  • Too tight compresses the product and pushes liquid out
  • Too loose creates gaps at the legs and waist where liquid escapes before it’s absorbed

The leg openings are the most common leak point. Adult diapers have leg guards to protect against leaks, if these aren’t properly activated, leaks are far more likely. This happens when the product is too large and the guards can’t sit flush against the skin.

Measure waist and hip size before buying, don’t guess. Choosing a larger size under the belief it will absorb more is a common mistake. In reality, an oversized product may slip, sag, or droop, causing discomfort and leaks. 

2. The Absorbency Isn’t High Enough

The main cause of leaks among adults is poor absorbency of the incontinence product. This is especially true overnight, when the product needs to handle several hours of use without a change. 

Daytime disposable adult briefs are not built for overnight use. They hit their absorption limit faster, and once saturated, liquid has nowhere to go.

For overnight use, switch to overnight incontinence underwear specifically. Overnight diapers are designed to be more absorbent than regular adult incontinence products, allowing for a full night of improved protection. Look for products with a breathable back sheet, inner leg cuffs, and a top liner that wicks moisture away from the skin. 

3. The Product Type Doesn’t Match the Situation

Pull-on adult disposable underwear and tabbed briefs are not interchangeable. Each suits different needs:

  • Pull-on / adult disposable underwear – best for mobile, active users; easier to manage independently
  • Tabbed disposable adult briefs – better for those with limited mobility or heavier incontinence; adjustable fit reduces gap leaks
  • Overnight incontinence underwear – higher absorbency core, designed for 8+ hours of continuous wear

Using a light daytime product through the night, or a pull-on for someone largely bedridden, will almost always result in leaks.

4. Wearing It Too Long

Although overnight diapers for adults have enhanced absorbency for long hours, not changing them from time to time can result in leakage. Wearing a diaper for too long can also bring about painful skin rashes, bad odor, and leaks due to soiling. 

Some products include a wetness indicator that changes color when the product needs changing, useful for caregivers who don’t want to disturb a sleeping person unnecessarily.

5. Not Using a Booster Pad When Needed

For heavier incontinence or extra-long wear, a booster pad placed inside the diaper extends absorbency without needing to size up. A booster pad is a permeable absorbency sheet placed in the middle of an adult diaper. The pad increases capacity by absorbing the first few incontinence episodes, once saturated, it allows urine to pass through to be absorbed by the diaper underneath. 

Note: use booster pads, not incontinence pads. Incontinence pads have plastic bottoms that do not allow urine to pass through, which results in the diaper not absorbing urine evenly and leads to leaks. 

What to Look for in a Breathable Adult Diaper

Beyond leak prevention, the material matters, especially for long wear. Non-breathable products trap heat and moisture against the skin, leading to irritation and rashes.

Look for

  • A breathable outer layer that allows airflow without compromising leak protection
  • A fast-wicking inner liner that pulls moisture away from the skin immediately
  • Soft, latex-free materials, especially important for sensitive skin
  • Leg cuffs that sit flush without digging in

The best overnight products retain higher volumes of fluid while trapping moisture inside, away from the skin. That combination of high absorbency plus skin-safe design is what separates a reliable product from one that leaks and irritates.
